EFPC Webinar November 17

The European Forum for Primary Care invites to a webinar:

Peter Groenewegen executive board member of EFPC and professor at the Netherlands Inst Health Service Research (NIVEL), will present a webinar, titled Primary Care Models for Tackling Multimorbidity. In this webinar Peter will address the problem of fragmentation of health care. He will present information on programmes in EU countries to integrate care for patients with multiple chronic diseases, based on information collected in an European study – the ICARE4EU study. The potential for improvement according to patients in the important areas of continuity, comprehensiveness and patient involvement will be presented on the basis of the QUALICOPC study. Finally, Peter will touch upon the issue of the transferability of practices from one country to another.

International Conference on Integrated Care

ICIC17- in Dublin
